Initial Medical Exam

The client must first be cleared to be treated with Buprenorphine or, in some cases, a short regimen of methadone. Physicians must be confident in the client’s ability to remain stable while being weaned from heroin and going through the induction process.

Choosing the Right Regimen

Medications commonly used in heroin detox include methadone, buprenorphine, and a combo of other substances to provide relief from acute aches and pains. Methadone and buprenorphine are opioid agonists that help reduce withdrawal symptoms and cravings. Although in this example, they’re being used for detox, they can also be used as a temporary but longer-term maintenance option.

The Induction

This is when the doctor physically introduces the buprenorphine into your system. Because of its sensitivity, and the way it interacts with opioid receptors, you must be clean from any opioid/opiate use for at least 12-24 hours. However, it’s worth noting that it’s not uncommon for some users to require 48 hours of no opiate or opioid use to begin induction. Little by little, your dose will be increased until we are certain you won’t experience precipitated withdrawals.

Stabilization

After you reach a stable dose, you’ll prepare for tapering. However, you must remain stable for a few days before we can begin to decrease your daily intake. Tapering

The tapering process occurs when we gradually decrease your daily intake of buprenorphine until you are at zero and completely free of any opioids, opiates, or agonists.
